3. Download the Access Request Form. You can find that HERE

4. You will also need to download the Supporting Evidence Form (SEF), if you do not already have supporting documents (this is on the same page as the access request form). 

5. You will need to work with your GP, allied health, and specialist medical professionals to prepare and provide the information required.

6. Return your supporting documents and request form to NAT@ndis.gov.au, 

7. The NDIS will then contact within the coming weeks to discuss your request.
